    <td><p>The Tierra del Sol Foundation, as a leading provider of independent living, job training and support services for people with developmental disabilities, is in need of a customer-focused Lead Maintenance Facilitator to provide oversight of maintenance, custodial and grounds personnel and oversee entire maintenance program for all facilities (7-acre Sunland campus, Foothill Office, Van Nuys Office, Claremont Art Gallery). This person will ensure all facilities, vehicles, etc. meet with Tierra Del Sol’s high standards. <li>Walk the property daily for trash and clean the granite signs as needed.  Clean the mailbox monthly.  Empty the cigarette disposal monthly.</li>
<li>Keep the garage and the maintenance office clean and orderly at all times.</li>
<li>Maintain security by routinely checking locks (repair and replace locks as needed), door jams, lighting and windows.</li>
<li>Schedule quarterly building review with appropriate staff.  After safety committees meets the Business Manager will provide you a report that you must return with notes and dates items were repaired with in thirty business days.</li>
<li>Open Sunland campus school buildings daily.</li>
<li>Provide minor repair and maintenance of heating and air conditioning units. Schedule filter replacement or cleaning, regularly assessing their performance and communicating concerns directly to vendor management.</li>
<li>Schedule routine landscaping maintenance: mowing, trimming, weeding, and planting. Contract large projects: tree removal, fencing, etc.  Provide scope of large projects to Business Manager for contracting.</li>
<li>Schedule routine rodent and pest control. Inspect for rodents, termites, black widows, ants, etc.</li>
<li>Ensure pool is properly maintained and secure.</li>
<li>Schedule routine gutter and drain maintenance on exterior of buildings.</li>
<li>Ensure lighting meets OSHA, fire department and security guidelines by scheduling routine checks of lighting at exits, pathways, offices, etc. Ensure circuits are not overloaded.</li>
<li>Schedule and maintain fire suppression systems. Fire suppression and alarm systems must be certified annually; fire extinguisher recharged annually and checked monthly, smoke detectors checked semi-annually.</li>
<li>Provide support to kitchen by checking water softener weekly and filling as needed.  Order salt as monthly.</li>
<li>Carry out vehicle fleet maintenance and repairs and ensure vehicles are equipped with fire extinguishers, flares or triangles, first aid kits, proof of insurance, valid registration and accident report. Inspect all vehicles for safety monthly.</li>
<li>Working with the Accounting Assistant, implement and maintain a Vehicle Fleet Management system such that all pertinent vehicle history is easily accessible electronically.</li>
<li>Order maintenance and vending machine supplies as needed using best price practices.  Restock as necessary.</li>
<li>Inspect all buildings, sidewalks, exits, and fire extinguishers monthly for possible repair or replacement including all offsite locations.</li>
<li>Report any unsafe or hazardous condition to Business Manager via email ASAP.</li>
<li>Communicate regularly with Business Manager to ensure appropriate prioritization of responsibilities<strong>.</strong></li>
<li>Provide guidance and direction to any part-time or volunteer maintenance staff.</li>
<li>Minor electrical repairs – Light bulbs replaced, ballast or fixture replacement, air conditioner fuses replaced, socket, switch or breaker repair, etc.</li>
<li>Perform minor short tasks as requested by Program Directors or Supervisors prioritizing, if necessary with Business Manager.</li>
<li>Ability to communicate with company-provided cell phone during regularly scheduled work hours.</li>
<li>Ability to effectively communicate using email systems.</li>

<p><strong>Qualifications: </strong> Must have a High School diploma or GED, Five or more years of performing maintenance on vehicle and/or equipment and/or five or more years experience working in general maintenance capacity or as a general contractor. Ability to speak with other staff, and supervisors in English as well as read, write and speak English, Spanish would be helpful but not necessary. Experience in heating and air conditioning units. The requirements listed below are representative of the knowledge, skill and/or ability required.  Must have a California driver’s license, good driving record and a minimum of 3 years driving experience and a current First Aid/CPR certificate.<strong><br />

<p>The PCSS/ILS/Vantage services Program Director leads a team of direct support staff and supervisors in developing and supporting inclusive community life styles, incorporating volunteer work opportunities, as well as social /recreational and educational opportunities for individuals with intellectual and other disabilities. The Program Director will directly supervise program managers and indirectly supervise direct support staff. The Program Director will also be responsible for effectively marketing and expanding all agency personalized services.</p>
<p><strong>Principal Areas of Responsibility:</strong></p>

<li>Develop and sustain positive working partnerships with community volunteer sites, education facilities, cultural, social-recreational and civic organizations.  The Program Director is responsible for developing a range of community partners sufficient to meet the needs of program participants.  The Program Director is responsible for establishing on-going Quality Assurance systems for each community partner sufficient to ensure that our staff and consumers are meeting the stated mission and expectations of each partner.</li>
<li>Develop and sustain positive working relationships with referral, regulatory, and collaborating social service agencies.  The Program Director is responsible for creating on-going communication and quality assurance systems that meet the highest expectations of all involved agencies.  The Program Director is responsible for communication with, and responsiveness to key agencies, including the Regional Centers, and medical professionals/clinicians who also serve our consumers.</li>
<li>Develop and sustain positive working relationships with consumer families and other key individuals who play an active and critical role in the consumer’s success in the PCSS/ILS/Vantage programs.  The Program Director is responsible for creating and sustaining systems of communication and collaboration that insure a high degree of consensus among all key partners regarding the consumers’ goals, objectives and action plans within these programs.</li>
<li>Develop and sustain a positive and effective team of staff to carry out the service mission of the PCSS/ILS/Vantage programs.  The Program Director is responsible for providing the leadership, management, training, support, supervision and evaluation that ensures that all PCSS/ILS/Vantage personnel understand the mission of the programs and possess all of the requisite skills and knowledge to successfully fulfill their individual roles within the program.  The Program Director is responsible for establishing and sustaining routine quality assurance programs that identify potential staff deficiencies in time to provide correction / training / support, or progressive discipline measures.</li>
<li>Develop and sustain systems for assessment of consumer needs, interests and capacities and for assessing the PCSS/ILS/Vantage programs’ effectiveness in assisting consumers in meeting their personal goals and objectives.  The Program Director is responsible for overseeing an intake, acceptance and initial assessment process, which identifies appropriate consumers and leads to the development of an accurate and actionable person centered service plan.  The Program Director is responsible for establishing and sustaining systems that effectively deliver the services identified in the person centered plan, and that consistently and accurately report consumer progress and performance as measured against person centered plan statements.  The Program Director is responsible for sustaining systems that routinely and accurately report emerging consumer problems, case management issues, or any significant changes in the consumer’s well-being that are apparent to staff.</li>

<li>Develop and sustain a positive and effective team of staff to carry out the service mission of the PCSS/ILS/Vantage programs.  The Program Director is responsible for providing the leadership, management, training, support, supervision and evaluation that ensures that all PCSS/ILS/Vantage personnel understand the mission of the programs and possess all of the requisite skills and knowledge to successfully fulfill their individual roles within the program.  The Program Director is responsible for establishing and sustaining routine quality assurance programs that identify potential staff deficiencies in time to provide correction / training / support, or progressive discipline measures.</li>

<p>This is an exempt position with typical hours of 7:30 a.m. – 4:30 p.m. Monday through Friday, with a daily "on-call" lunch period. Weekend and evening hours may be required to support staff who provide non-traditional service hours for clients.</p>
<p><strong>Qualifications: </strong></p>

<li>Bachelor’s degree in the field of Social Services required, in addition to experience in providing coordination of services for adults with developmental disabilities, and experience with staff training/supervision.</li>

<p>The Community Service Coach is under the supervision of the Program Director and Area Supervisor, and has the primary responsibility of supporting and training clients in the areas of community volunteerism, access, inclusion, and relationship building.</p>

<li>Demonstrate an understanding of the “big picture” and long-term outcomes for clients, with an understanding that an intensive intervention process may need to be implemented, rather than settling for a “quick fix”.</li>
<li>Demonstrate an ability to assess and problem solves a situation in the moment, using good judgment and common sense, without having to seek guidance/advice.</li>
<li>Accept, and effectively process directions, constructive feedback, and training from any leadership/mentor staff, then demonstrate in action/written product that this was internalized.</li>
<li>Assist in the development/implementation of an Individual Service Plan for each client based on information gathered through Person Centered Planning meetings.</li>
<li>Plan, implement, and support activities/routines with clients, which will assist them in achieving their Individual Service Plan objectives.</li>
<li>Document client progress through assessment/data collection instruments.</li>
<li>Complete data, progress notes, schedules, take client attendance accurately, and turn in on time, according to supervisor expectations.</li>
<li>Maintain conscientious, responsible level of supervision for clients to ensure their safety.</li>
<li>Assist with medical, behavioral, and other client emergencies.</li>
<li>Respectfully assist clients with personal care/mobility/toileting/feeding needs as applicable.</li>
<li>Encourage age-appropriate dress, grooming, and hygiene, which meet community standards.</li>
<li>Facilitate client self-advocacy and choice-making opportunities.</li>
<li>Assist clients in safely accessing a variety of community locations using public transportation, company, or personal vehicles.</li>
<li>Work cooperatively with clients’ families/care providers to foster client success.</li>
<li>Develop community resources for clients, such as volunteer jobs, and other opportunities for relationship building.</li>
<li>Ensure timely arrival to volunteer jobs, high level of motivation for completion of tasks, and actively support relationship building for clients.</li>
<li>Ensure that all assigned volunteer duties are consistently completed according to volunteer site manager standards and expectations.</li>
<li>Maintain regular communication/build positive partnership with volunteer site manager.</li>
<li>Communicate client issues to Area Supervisor and program/staff issues to Program Director.</li>
<li>Attend all in-services and staff meetings and perform all duties as assigned by Program Director.</li>
<li>This position is currently scheduled at 40 hours per week, with typical work hours of 7:30 a.m. – 3:30 p.m. Monday through Friday. These hours and duties are subject to change.</li>

<p><strong>Qualifications: </strong></p>
<ul>
<li>Minimum of 2 years’ college education, and experience in providing training/support to individuals with developmental disabilities.</li>
<li>Current, valid California driver’s license, good driving record, 3 years driving experience, vehicle insurance, and personal vehicle in roadworthy condition (with seatbelts to accommodate 3- 4 clients) for use in regularly transporting clients to community locations.</li>
<li>Working knowledge of task analysis/assessment/positive behavior supports (PBS) and clear written and verbal communication skills.</li>
<li>Must be able to pass a pre-employment physical &#38; drug test, DOJ LiveScan background investigation, and safe driver road test.</li>

<p><strong>Physical Requirements of the Position:</strong></p>

The Employee may also be required to carry a backpack that should not weigh more than 10 lbs. which will be provided to you by Tierra Del Sol. Backpacks allow us to carry all necessary information and supplies in a <span style="text-decoration: underline;">hands-free manner</span> which optimizes safety for our clients when out in the community. Specific vision abilities required by this job include close/distance/color vision, depth perception, and the ability to adjust focus. Ability to physically assist disabled adults weighing up to 200 pounds in the use of community transportation, i.e. boarding buses, vans, transferring from wheelchair to vehicle/toilet, and/or pushing a wheelchair when accessing the community. This may also include assisting clients with feeding/personal care needs. Physical/cognitive ability to effectively utilize Pro-ACT techniques in response to behavioral emergencies, and First Aid/CPR in medical emergencies is necessary.</p>

<p><strong>Position:</strong>  Community Service Coach for Adults with Intellectual Disabilities<strong>  </strong></p>
<p><strong>Location:</strong>  San Fernando Valley (could be at Pierce College, Valley College, or Mission College, depending upon client’s needs)</p>

<p>The Community Service Coach is under the supervision of the Program Director and Area Supervisor, and has the primary responsibility of supporting and training clients in the areas of community volunteerism, access, inclusion, and relationship building.</p>
<p>R<strong>esponsibilities include, but are not limited to:</strong></p>

<li>Establish positive, supportive, respectful relationships/interactions with clients.</li>
<li>Become knowledgeable about the clients’ interests, unique abilities and support needs.</li>
<li>Demonstrate motivation and enthusiasm in all interactions with clients.</li>
<li>Encourage active participation of clients in all activities (allow them to do as much for themselves as possible).</li>
<li>Interact with community members in a way that encourages respectful, age-appropriate treatment of clients, and the building of mutually respectful relationships.</li>
<li>Maintain a sense of calmness/maturity/professionalism when interacting with clients, co-workers, and community members.</li>
<li>Clearly communicate with supervisors/co-workers, and pro-actively address questions/concerns.</li>
<li>Develop positive/respectful relationships with families, and outside agency staff. This includes the ability to be empathetic and sensitive to issues/stressors/external factors, which may impede the parent and care provider’s ability to support clients in a way, which matches Tierra’s philosophy. This also includes the ability to be assertive, and to advocate for client needs/rights without being aggressive.</li>
<li>Demonstrate an understanding of the “big picture” and long-term outcomes for clients, with an understanding that an intensive intervention process may need to be implemented, rather than settling for a “quick fix”.</li>
<li>Demonstrate an ability to assess and problem solves a situation in the moment, using good judgment and common sense, without having to seek guidance/advice.</li>
<li>Accept, and effectively process directions, constructive feedback, and training from any leadership/mentor staff, then demonstrate in action/written product that this was internalized.</li>
<li>Assist in the development/implementation of an Individual Service Plan for each client based on information gathered through Person Centered Planning meetings.</li>
<li>Plan, implement, and support activities/routines with clients, which will assist them in achieving their Individual Service Plan objectives.</li>
<li>Document client progress through assessment/data collection instruments.</li>
<li>Complete data, progress notes, schedules, take client attendance accurately, and turn in on time, according to supervisor expectations.</li>
<li>Maintain conscientious, responsible level of supervision for clients to ensure their safety.</li>
<li>Assist with medical, behavioral, and other client emergencies.</li>
<li>Respectfully assist clients with personal care/mobility/toileting/feeding needs as applicable.</li>
<li>Encourage age-appropriate dress, grooming, and hygiene, which meet community standards.</li>
<li>Facilitate client self-advocacy and choice-making opportunities.</li>
<li>Assist clients in safely accessing a variety of community locations using public transportation, company, or personal vehicles.</li>
<li>Work cooperatively with clients’ families/care providers to foster client success.</li>
<li>Develop community resources for clients, such as volunteer jobs, and other opportunities for relationship building.</li>
<li>Ensure timely arrival to volunteer jobs, high level of motivation for completion of tasks, and actively support relationship building for clients.</li>
<li>Ensure that all assigned volunteer duties are consistently completed according to volunteer site manager standards and expectations.</li>
<li>Maintain regular communication/build positive partnership with volunteer site manager.</li>
<li>Communicate client issues to Area Supervisor and program/staff issues to Program Director.</li>
<li>Attend all in-services and staff meetings and perform all duties as assigned by Program Director.</li>
<li>This position is currently scheduled at 40 hours per week, with typical work hours of 7:30 a.m. – 3:30 p.m. Monday through Friday. These hours and duties are subject to change.</li>
</ul>
<p><strong>Qualifications: </strong></p>
<ul>
<li>Minimum of 2 years’ college education, and experience in providing training/support to individuals with developmental disabilities.</li>
<li>Current, valid California driver’s license, good driving record, 3 years driving experience, vehicle insurance, and personal vehicle in roadworthy condition (with seatbelts to accommodate 3- 4 clients) for use in regularly transporting clients to community locations.</li>
<li>Working knowledge of task analysis/assessment/positive behavior supports (PBS) and clear written and verbal communication skills.</li>
<li>Must be able to pass a pre-employment physical &amp; drug test, DOJ LiveScan background investigation, and safe driver road test.</li>
</ul>
<p><strong>Physical Requirements of the Position:</strong></p>

The Employee may also be required to carry a backpack that should not weigh more than 10 lbs. which will be provided to you by Tierra Del Sol. Backpacks allow us to carry all necessary information and supplies in a <span style="text-decoration: underline;">hands-free manner</span> which optimizes safety for our clients when out in the community. Specific vision abilities required by this job include close/distance/color vision, depth perception, and the ability to adjust focus. Ability to physically assist disabled adults weighing up to 200 pounds in the use of community transportation, i.e. boarding buses, vans, transferring from wheelchair to vehicle/toilet, and/or pushing a wheelchair when accessing the community. This may also include assisting clients with feeding/personal care needs. Physical/cognitive ability to effectively utilize Pro-ACT techniques in response to behavioral emergencies, and First Aid/CPR in medical emergencies is necessary.</p>
